Exploring A Trip To Rome/Pompeii

Tossing around the idea of traveling to Rome, Pompeii, and a couple other historic areas for a trip. My wife has taken an interest in a history show I watch about the ancient Romans and that time era. Looking at some stuff it seems most of these areas are open for tourists.

I have never planned a trip abroad before, would be traveling from the US. What are some tips for planning such a trip?

Hotel stays, transportation, guides (if needed)? Mostly nervous about transportation and if there's going any communication issues with only speaking English?
